1. Added 2-factor authentication.

This is a big change that improves security and should be used by all users eventually. It is off by default, but can be enabled per-user in Personnel and when users edit their profile.  When enabled it sends an email to the user at log-in for an authentication code with the option to stay logged in for 60 days.  2-factor will need to be authenticated on each new browser (phone, computer, etc).

Most users are familiar with 2-factor from gmail, etc and while it's optional, if you are storing sensitive information and documents in Luigi you should consider enabling it. I can also enable it site-wide for your organization, so please give me some feedback as you test it.

2. --CONTACTWITHROLES-- variable now will display all folks on the repertoire tab, whether or not they are assigned to pieces or added manually. This will make it more useful for creating personnel lists through the Production tab.
